Forms and information can be found at these links! You can download, print and return the completed forms to the church office, unless otherwise noted.
Check here throughout the year for forms to attend church events (i.e. car show, corn hole tournament, Oktoberfest) and to show your interest in a church project (i.e. columbarium). You also can make donations/payments online or by mobile app! Click on this link to our Giving page to learn more.
Member Parking Form
Parking stickers are free to members and are required so that your car is not towed from the lot on the west side (back) of the church. Print and complete the form at the link above and bring it to the church office. Office staff keep the forms secure and confidential. Stickers should be placed on the inside of the front windshield, lower corner, PASSENGER side. Be sure to request a new sticker if you replace your windshield or vehicle.
Simply Giving Form
Simply Giving allows members to automatically set up their church giving. Please download, print and fill out the form and return it to either Debbie Downey or the church office.

RE:Formation Form
We are always looking for ways to connect people in our community to each other and to God. Use this form to share an idea or a need in our community. Completed forms may be brought to the church office or emailed to Deacon Donna Summerville.
Church Expense Reimbursement Form
This form is for church members to fill out for reimbursement. The form is designed so that you can type into the appropriate blanks, print the result and take it to the appropriate committee chair. You can see a filled out example to guide you.filled out example

This form is for anyone who wants to put their wishes on paper for the church and loved ones to use when our time comes.
